Volume 1 provides you with essential end-of-life planning information and this companion volume (Vol. 2), contains forms for you to fill in your personal information, preferences, and insights. We have designed volume 2 to have forms for two people sharing a household and some forms can be completed together if the information would be the same.

The ideal way to use the two volumes together is to open up volume 1 to the chapter you are working on, and then beside it, open up the corresponding forms for that chapter in volume 2 so you can refer back and forth between the two as you work. Start with the chapter you feel ready to tackle or the one you feel is a priority. There is no need to complete them sequentially. Our books are comprehensive, and it will take some time to complete, but it is our hope that fulfilling this project will bring you peace from knowing you have enabled your loved ones to meet the many challenges that could arise in your absence.
